CONSENT TO CALL RECORDING

with the date, the time and the length of the call.

PURPOSES. Tick the ones you agree to. You may agree to one without agreeing
to the others.

Quebec: we have carried out the assessment required before that communication.]

Withdrawal applies going forward.

ACCESS AND CORRECTION. You may ask for a copy of what we hold about you, or
for it to be corrected, at the same address.
